The Bellingham Patient Navigator will work with clients at the medical clinic in Bellingham, WA. The Bellingham Medical Clinic specializes in family medicine and providing health care for the entire family and offers a wide range of acute, chronic, and preventive medical services. The Patient Navigator will work with various health educators to assist patients in accessing appropriate care, establishing a medical home, ensuring their pre- and post-visit needs are met and facilitating brief health education encounters. The navigator will also participate in a variety of health education opportunities, including facilitation of classes, and one on one meetings with patients. The navigator will also have opportunities to shadow providers on-site.

A major team component is also required of the Patient Navigator. This includes bi-weekly team meetings and trainings, as well as service projects conducted as a team. The Sea Mar Community HealthCorps team consists of 24 members including the Bellingham Patient Navigator.
