The Chronic Disease Self-management program, is a workshop given two and a half hours, once a week for six weeks. Workshops are facilitated by two trained leaders.
This is a evidence based program with subjects that cover chronic diseases that allow people with different health problems attend together.
Classes are highly participative, where mutual support and success build the participants confidence in their ability to manage their health and maintain active fulfilling lives.

The mission of the Bucks County Area Agency on Aging is to promote the engagement of volunteers to assist with seniors and senior needs in the community. Positively Aging Bucks County Volunteer Program is to focus on community improvement and in the delivery of volunteer services to seniors in Bucks County .
Description
The Bucks County Positively Aging volunteer program provides opportunities for adults age 18+ to engage in volunteer services and contribute time,experience and knowledge to others in their comuunities.
