Do you want to take CONTROL OF YOUR HEALTH? Do you want to make a choice in your community? Get involved in disease SELF-management education!

Community Choice offers Chronic Pain Self-Management, Chronic Disease Self-Management and Diabetes Self-Management workshops and trainings. Individuals can get involved in two ways: as a participant of our workshops and as a Lay Leader (individuals may choose to participate in both). Lay Leaders are those who get training to learn how to teach/facilitate workshops. Participants, on the other hand, are those who attend said workshops. Based on our most immediate scheduling, we are currently recruiting participants of the Diabetes Self-Management Program in English AND Spanish, as well as Chronic Pain Self-Management.

Involvement opportunities:

Leader training (to become a workshop facilitator):
-Trainings are in English and Spanish for most of the programs above (Chronic Pain in Spanish may be offered by the end of 2016).
-Each training is 32 hours, divided into four days.
-We provide the cost of the Leader Certification.
-We provide the food during training.
-We provide materials (including the Leader's Manual).
-To maintain certification as a Lay Leader, individuals will teach one workshop a year after have received the certification.

Workshops:
-Offered in English and Spanish f or most of the programs above.

-Duration: 6 week program which meets once a week for 2.5 hours.

-We provide the cost of the course.
-We provide snacks during each lesson.


Description of the Diabetes Self-Management program

Topics covered:
1) techniques to deal with the symptoms of diabetes, fatigue, pain, hyper/hypoglycemia, stress, and emotional problems such as depression, anger, fear and frustration; 2) appropriate exercise for maintaining and improving strength and endurance; 3) healthy eating 4) appropriate use of medication; and 5) working more effectively with health care providers.

Participants will make weekly action plans, share experiences, and help each other solve problems they encounter in creating and carrying out their self-management program. Physicians, diabetes educators, dietitians, and other health professionals both at Stanford and in the community have reviewed all materials in the workshop.

This program is excellent both for health professionals or caregivers. Also, continuing education credits (CEUs) are available for health professionals. Students will learn to apply techniques conducive to significant improvements in depression, symptoms of hypoglycemia, communication with physicians, healthy eating, and reading food labels as well as patient activation and self-efficacy.

The Diabetes Self-Management Program, developed by Stanford University Patient Education Research Center, is an evidence and research-based program that has been shown to reduce hospitalizations and improve quality of life.

1. Date: September 2nd - October 7th (every Wednesday for 6 weeks)

Time: 5:00PM - 7:30PM

2. Date: Oct. 10 - Nov. 14 (every Saturday for 6 weeks)

Time: 9:00AM - 11:30AM

3. Date: Oct 22 - Dec 3 (every Thursday for 6 weeks) *skipping November 26th for Thanksgiving

Time: 12:00PM -3PM

4. Date: Nov 6 - Dec 11 (every Friday for 6 weeks)

Time: 9:00AM - 11:30AM

Upcoming Chronic Pain Self-Management:

Date:Sept. 10 - Oct. 15 th (every Thursday for 6 weeks)

Time: 6:00PM to 8:30PM

Upcoming Chronic Disease Self-Managemente Leader Training:

Our upcoming training will be held on Sept. 19, 20 and Oct. 3 & 4.

Leaders receive $200 for every facilitated workshop
