Compassionate Care Hospice Team
CCH Clinical Team and Volunteers for 2015 National Volunteer week activities

Patient Care Volunteer: provide compassionate companionship and emotional support for Hospice patients and their families. Some examples of what our volunteers have or are doing: conversation & Deep Active Listening ("I'm so angry/sad/scared I'm dying"; "I'm deeply spiritual and need someone to talk to about what's happening to me now"); culturally diverse activities - music (discussing, playing Bolero music/songs for Puerto Rican patients, Gospel for Baptist patients, opera for Italian patients); craft activities w/ children (memory/legacy book for grandchildren of patient; games, drawing music activities w/ patient's children); spiritual conversation, activities (discussing Koran, end of life beliefs w/ Muslim patient and family, bible study w/ Christian patients); "comfort" activities (singing, poetry, music, painting nails/brushing hair, holding hands, saying "I love you"); celebrate! (holidays, birthdays, anniversaries, decorate rooms).

Music Therapy Volunteer: Use your voice or musical talent to heal and comfort.

Pet Therapy Volunteer: Your team of you and your dog will make "therapy" visits with our Hospice patients.

Art Therapy Volunteer: Use your drawing, painting, sculpting, graphic arts skills to engage Hospice patients or provide legacies for family members.

Alternative Therapy Volunteer: Use your special passion and skill to support Hospice patients and their families:

Massage Therapy

Accupuncture

Reiki / Energy Therapy

Yoga / Meditation

Aroma & Sound Therapy

Office / Administrative Volunteer: assist in the day-to-day administrative tasks of a manged care Hospice operation

Bereavement Volunteer: assist surviving family with loss and grief issues; bereavement mailings; assist chaplain, bereavement coordinator or social worker with psycho-social, emotional, spiritual support.

End of Life Support for Veterans: "Veteran-centric" end-of-life support. CCH is a Level Four (highest level) participant in the Hospice-Veterans Partnership and We Honor Veterans Program.

Comprehensive training and supervision provided for all Volunteer positions.

Compassionate Care Hospice's mission statement is: To affirm life during its final stages by providing holistic compassionate care to patients and their families; by providing access to hospice (comfort) care for under-served or difficult to serve populations; by compassionate and efficient use of available resources, and by sensitively educating the community.

Description

Hospice affirms life and does not hasten or postpone death.

The hospice philosophy of care accepts death as a natural, final stage of life. It affirms life and focuses on quality rather than length of life. The goal of hospice is to enable patients to continue an alert, pain-free life and to manage other symptoms so that their last days may be spent with dignity and quality, surrounded by their loved ones.

Hospice care treats the person rather than the disease.
Hospice patients have a terminal illness with a prognosis of less than 6 months and have chosen to receive " palliative" as opposed to " curative" care. Focus is on maintainng and enhancing comfort, dignity and quality of life for both patient and family/caregivers during the end-of-life process, holistically treating the symtoms of the terminal illness by providing support for medical/physical, psycho social, spirital and other pain and suffering, attempting to respect the dignity of the person as a human being.

Hospice care is family-centered care.
It involves the patient and the family in making decisions. Care is provided for the patient and family 24 hours a day, 7 days a week.
