Volunteers will review current medication lists, offer support and education to clients via phone or in office. Volunteers will be supervised by the Nurse Case Manager.
Specific Tasks include:
Educating Patient about the medications and dosage form.
-Checking for contraindications and adverse effects
-Checking the expiration date and storage procedure for the medication.
-Analyzing patient's health condition and over the counter drugs.
-Clarifying patient's questions and providing necessary information.
-Follow up service.
