Connection to Care Advocate with Mobility Management of South Central NY

The Connection to Care Advocate will serve with a team of transportation professionals and AmeriCorps members to improve access to health care and other essential services for people who face barriers to care, including the elderly and people with disabilities. Mobility Management of South Central New York (MMSCNY), located at United Way of Broome County, operates the GetThere Call Center, which serves as a one-stop transportation assistance center, providing case management, information and referral services, trip planning, travel training, and transportation education services. The Connection to Care Advocate will provide one-on-one assistance to participants, and will work collaboratively with the health and human services community to formulate solutions to participants’ transportation needs. In addition, the member will outline strategies to improve awareness and utilization of MMSCNY services through community outreach, marketing/ communications, publication development and media relations. Opportunity to practice analytical and problem-solving skills, gain experience in the health and human services sector, and play an integral role in the success of a dynamic community initiative.

- Complete 900 or 1700 service hours between November/December 2016 - October 2017 (a full and part-time position is available)
- 35-40 hours weekly (full-time) or ~20 hours weekly (part-time)
- Living Allowance of $12,530 (full-time) or $6,050 (part-time)
- Segal Education Award of $5,775 (full-time) or $2,822 (part-time)
- College loan forbearance available
- No-cost health insurance available for full-time member

The purpose of the Rural Health Service Corps is to provide meaningful service and learning opportunities for young people committed to improving the health and lives of those living in South Central New York State.
The mission of the Rural Health Network of South Central New York (RHNSCNY) is to promote and improve the health of rural communities and strengthen each community's capacity to respond to health needs by reducing barriers and improving access and use of health and human services through advocacy, communication and partnerships. The RHNSCNY serves Delaware, Tioga and rural Broome County.
