Canyon River Community Hospice offers a wide variety of volunteer opportunities. From providing companionship to patients, assisting with administrative tasks in our corporate offices, to making posts to our Facebook page, our volunteer needs are wide and varied.

Each of our volunteers benefit from our extensive training program which ensures each new volunteer is confident, assured, and well-prepared to support the local community in which we serve. Your efforts are greatly appreciated--and as such, we offer flexibility needed to meet the scheduling needs of each of our volunteers.

At Canyon River Community Hospice our mission is a simple one -- that being to focus on assisting each patient 'enjoy the moments’. Helping patients enjoy the moments by controlling pain and handling symptom management, through education and preparation, by 'just being there’ to support and to comfort.

Description

At Canyon River Community Hospice, we are dedicated to providing the care and compassion our patients and their families deserve. With years of hands-on hospice experience, our team of caring physicians, nurses, volunteers and social workers provide each Canyon River Community Hospice 'community member’ with the support that enables patients to go undergo the dying process with dignity and the families to cope with loss with strength.

As a community-based hospice, we strive to be entrenched in the community to serve, to nurture, and to thrieve. And as such, we welcome the opportunity to share our message and our vision.

There are varying levels of care which are designed to meet the changing clinical needs of the patient.

By understanding the patient and family’s needs and preferences ad by conducting a clinical assessment, Canyon River Community Hospice determines what the most appropriate level of care will be.

Levels of care include:

- Routine Care
- Respite Care
- Inpatient Care
- Continuous Care

Routine Care

Routine care is general hospice care provided wherever the patient calls "home". This can include a private residence, nursing home, or other like facility. Routine hospice care includes pain and symptoms management, team support, and regular visits by hospice staff.

Respite Care

Respite care is made available to provide a break or reprieve to the family and caregivers when needed. Care is typically provided in approved hospitals or long-term care facilities and can also be supported in a hospice inpatient unit. Respite care may extend to a maximum of 5 days in a row.

Inpatient Care

Inpatient care is provided when a patient needs greater pain and symptom management than can be provided at home. Care is provided in approved hospitals or a dedicated hospice inpatient unit where patients can receive expert care on an around the clock basis.

Continuous Care

Continuous care if offered during periods of crisis in order to maintain a patient at home. This level of care is appropriate when symptoms are not well controlled and the patient requires nursing care to achieve some level of symptom control.
