• LifeLong Connect is a new service for LifeLong Medical Care patients that connects them directly to their health information. LifeLong Connect allows our patients to make appointments, renew prescriptions, review their personal health record, make payments, access health education, and request information through an online portal.
  
  
• Patient Portal Specialists will have the responsibility of outreaching, educating, and enrolling our patients into LifeLong Connect. All interactions will be one-on-one and in-person. Specialists will also work directly with health center clinical support staff.
  
  
• LifeLong Medical Care is seeking dynamic volunteers with a passion for community health. The ideal candidates for this position will have:
  • Strong work ethic and ability to prioritize multiple tasks and follow through in a timely manner
  • Ability to work independently as well as an effective team member
  • Attention to detail and accuracy
  • Excellent listening, written, and verbal communication skills
  • Initiative to ask questions and make proposals
  • Excellent customer service skills
  • Ability working with underserved and/or underrepresented, diverse populations
  • Proficiency in Windows computer systems and Chrome
  • Experience in training different age groups how to use online systems
    
    
• Location of two health centers participating in LifeLong Connect:
  • LifeLong Berkeley Primary Care health center, 2001 Dwight Way, Berkeley
  • LifeLong East Oakland health center, 10700 MacArthur Blvd., Oakland (driving is recommended)
    
    
• Time Commitment: Minimum of 4 hours per week, 3 months. Monday through Friday, between 8:30am-4:30pm.
