The Sunset Park community is a diverse, low-income community comprised of primarily recent immigrants. Sunset Park Community HealthCorps views health in a holistic way and recognizes the impact of issues such as literacy, housing, family violence, employment, and trauma on an individual's well-being. The programs in which HealthCorps members are placed reflect this view, and members work to provide patients with case management, patient navigation, referrals, program enrollment, and senior support services. Members receive training and orientation in the health centers and community programs of our parent organization, Lutheran Family Health Centers. Members are matched to individual placements throughout our family health centers and community programs. In their positions and as a team, members engage in health education, case management, supportive services, and program enrollment, among other services. Due to the large volume of applications we receive, we ask all applicants to contact our program by email after submitting an AmeriCorps application through the portal.

Members are matched to placements throughout our family health centers and community programs. Below you will kind a list of the programs we are currently recruiting for:

1. Case Management at Family Support Services (Full Time) - Bilingual in English and Mandarin required. AmeriCorps member will assist clients by helping them access public benefits, literacy, housing, domestic violence, employment, food stamps, public assistance, food pantries, immigration, childcare facilities, and legal resources through providing case management and supportive counseling. A background in Psychology, or Social work preferred. Located: 6025 6th Ave, BK NY 11220

2. Project Reach Youth (Full Time & Part Time) - Engage with youth and adult students between the ages of 16-24 for HSE tutoring. Ideal candidate would have a background in education or tutoring and public speaking. Project Reach Youth - 199 14th St, 3rd floor, Brooklyn, NY 11215.

3. Brooklyn Workforce Innovations - Brooklyn Woods (Full Time) - AmeriCorps member will help build the capacity of a job training program that teaches woodworking skills to low income New Yorkers with barriers to employment. AmeriCorps member will work on outreach, recruitment, screening candidates, post training support as well as general program support. Ideal candidate will have strong communication skills, willingness to take initiative, able to handle administrative duties. Located: 125 8th Street Brooklyn, NY 11215

Program benefits: Total education award is $12,530 ($1,135 per month), Education award upon successful completion of service is $5,370, training, childcare assistance if eligible, and Health Coverage.

Candidates:

-This is a 1700 hour, 10.5 month contact

-Must be 18 or over

-Must be a US citizen or legal permanent resident

-Must be able to work Monday - Friday at least 9am - 5pm

-Bilingual (preferred for most programs)

Lutheran Family Health Centers is a federally qualified health center network dedicated to meeting the health and human service needs of the underserved communities of southwest and central Brooklyn. Lutheran Family Health Centers(LFHC) Department of Community-Based Programs has provided educational and supportive services for community residents of southwest Brooklyn for over 35 years. Our work is grounded in the holistic philosophy that is the foundation of LFHC's orientation to community health - that health depends upon such factors as psychosocial, familial, economic, housing, educational, and employment status. We are committed to affirming each family's cultural, ethnic and linguistic identities and using community social work practices, to ensure that our programs are always responsive to evolving family and community issues in the context of a diverse, multicultural society.

Description

We offer the following areas of expertise:

  • Culturally competent programming that meets the needs of the diverse populations, particularly new immigrant families, in our community. These programs include: Adult Basic Education, GED-prep, English Classes, Family Support Services, Day Care Centers, After-school Programming, AmeriCorps Service Opportunities, HIV Peer Education, Youth Development, and Services for Older Adults.
  • Strong collaborative relationships with community partners to create integrated service delivery systems.
  • A model of community building, that promotes participatory management and supports relationship and community involvement at all levels.
  • Recognizing that families are resources to their own members, to other families, to programs and to communities.
  • Advocating for services and systems that are fair, responsive and accountable to the families served. Mobilizing formal and informal resources to support family development.
