Premier Hospice is in need of caring, compassionate people to provide friendly visits to patient's, and their families, living with a terminal illness. Volunteers offer companionship and support to patients by reading a book, playing cards, writing letters, going for walks, running errands, or assisting with other projects as requested by the patient and team. Premier Hospice Volunteers are the Heart of the Patient Care Team, offering companionship and support when it is needed the most.

Premier Hospice is committed to providing and promoting the highest quality of care to patients, and their families, facing end of life issues. Premier Hospice integrates Volunteers into the hospice program by actively recruiting, training, supporting, and utilizing Volunteers to meet the needs of the patient and family. Volunteers are a highly respected and valued member of the Patient Care Team.

Description

Premier Hospice provides hospice services to patient's with a terminal illness and a life expectancy of six (6) months or less.

Hospice services are available 24 hours a day, 7 days a week, (including Holidays), if needed, and are provided in the comfort of the patient's home, ("home" may be a skilled nursing facility, group home, assisted living complex, or a private residence).

A team of professionals, including a Physician, Nurse, Social Worker, Chaplain, Certified Nursing Assistant (CNA), and a Volunteer, is assigned to each patient, and will provide individualized care based on the needs of the patient and family.

Premier Hospice serves patients that reside in Maricopa County.
