Heartland Hospice is seeking retired professionals to offer one-on-one education to our patients and families. Further education on topics such as community resources, funeral planning, sacred presence at dying, anticipatory grief, and reflective writing will be offered to families through our volunteer program.

Mission Statement
The mission of Heartland Hospice is to provide quality hospice care to patients with a limited life expectancy, as well as, support to their caregivers. The quality of life for those we serve is enhanced through palliative and specialized supportive care designed to meet the physical, psychosocial, and spiritual needs of the patients and caregivers.
Description
Hospice care is provided by an interdisciplinary team consisting of the Medical Director, Primary Physician, Nurses, Social Workers, Chaplains, as well as Bereavement Coordinators and Volunteers. Care is delivered to patients either in their own home or extended care facility. Heartland Hospice strives to provide care to all who are eligible and need hospice care irrespective of ability to pay.
