National Stroke Association is looking for a Stroke Recovery Navigator Intern for Spring of 2015! The Stroke Recovery Navigator program is designed to fill a critical gap for stroke survivors and caregivers as they negotiate the complex world of post-stroke care. In numerous studies, survivors and caregivers have indicated that they feel abandoned and need more information about recovery and assistance in locating services.

The Stroke Recovery Navigator will enroll stroke survivors and caregivers at time of discharge from an acute care setting (hospital). Trained navigators will then proactively contact them, conduct an initial needs assessment, and then provide ongoing information, resources, and support to facilitate recovery.
