Title: Intake Volunteer Purpose: To welcome patients to Wallace Medical Concern front desk check-in area of the clinic. Responsibilities: Check in patients Make charts, screen Patient Intake Forms, and double check application. Explain Wallace Medical Concern’s protocols with walk ins and scheduled patients. Answer patient questions Data entry in EMR Filing and scanning Clean Waiting Room counters, tables, wipe down and put away toys in waiting room. Assist with volunteer mailings Other misc. administrative tasks Set up for intake. Put intake materials away at the end of clinic EPIC support in scheduling and registration of patients Timeframe: Length of commitment: minimum 6 months Estimated total hours: minimum of 4 volunteer shifts per month (approx. 15 hours) Volunteer Shift: Monday 8:30am-noon or 5:30pm-9pm Scheduling: done by volunteer manager every 2 months Worksite: 124 NE 181 st Suite 152 Portland, OR 97230 WMC Training Required: Attend 3 hour training on OCHIN EPIC at WMC or demonstrate EPIC proficiency.
