ORGANIZATION: Horizon Home Care and Hospice

They loved and cared for us for many years, now we can love and care for them by bringing quality-of-life to the end-of-life for our hospice patients and families. Horizon has three opportunities to serve in this very meaningful capacity. Choose the opportunity that best fits your interest and comfort level:

Friendly Visitor: Provide companionship and/or respite to in-home hospice patients and families. Our Volunteer Coordinator will thoughtfully assign you to a case based on your availability, personality and proximity (5 - 15 mile radius). If you desire, you may also offer additional cares by helping with dishes, errands, laundry, etc.

Hospice Companion: Assist nurses with direct patient cares (feeding, mouth-swabbing, repositioning, etc.) at the Horizon In-Patient Unit, Mequon. Volunteers serve a 3 - 4 hour shift and love getting the chance to connect with our patients, families and nurses on a weekly basis. If you want to have a sense of 'team’ this is the right position for you.

Final Hours Volunteer: Offer the ultimate presence to those who are in their final hours of life. Volunteers are on-call to provide 2 - 4 hours of love and support to both the patient and their loved ones. You will feel amazed and privileged to be a part of someone’s end of life journey.

Reach out to us today and we will get you started on this purposeful service experience. Please leave your phone number and email address so we can best reach out to you.Thank you. Make it a great day.

About Horizon Home Care and Hospice

Location:

11400 West Lake Drive, Milwaukee, WI 53224, US

Mission Statement

Horizon is a home care and hospice provider built on a foundation of care that celebrates the human spirit and the desire for security, comfort, and compassion. We are a professional team of 550+ employees and 150+ volunteers who make a loving difference in the lives of more than 2,500 patients and families throughout Southeastern Wisconsin. The idea of 'home' is central to Horizon. Home means different things to different people, but one thing it should always be is a place where we value each other. It's a place where people genuinely care about the comfort and security of others. We know our purpose is to give our patients home, whatever that means to them, wherever they are. More about Our Local Non-Profit Horizon Home Care & Hospice was established in 1990 and today serves as the second largest full service home care agency and hospice in Wisconsin. We are a local, non-profit organization sponsored by Froedtert Memorial Lutheran Hospital and Columbia St. Mary's Hospital.
