About Street Level Health Project
Location:
2501 International Blvd, Oakland, CA 94601, US
Mission Statement
Street Level Health Project is a nonprofit organization dedicated to improving the health and well-being of underserved urban immigrant communities in the Bay Area. Our organization is an entry point to the health care system for those most often overlooked and neglected, namely the
uninsured, underinsured, and recently arrived.
Street Level Health Project's model for closing the gap in health care access for low-income, recently immigrated communities is three-fold: 1) bringing health care to the community through our Health Access Program; 2) tightening the link between individuals and community health clinics through our Navigator Empowerment Program; and 3) addressing the root causes of poor health by strengthening and empowering individuals and communities to be healthy through our Health Education and Leadership Program.
SLHP develops trusting relationships with new immigrants, offers them a place to build a healthy and vibrant community, and empowers them to advocate for the well-being of themselves and their families.
Description
Health Screening Clinic
Street Level's free health screening clinic is open to all low-wage workers who do not have health insurance. We offer basic diagnostic and treatment services, as well as referral and case management services. Our services are available without regard to ethnicity, race, gender identity, sexual orientation, religion, immigration status or ability to pay.
Referrals and Health Navigation
Street Level provides free assistance to
clients in need of help navigating the local systems and services available for healthcare, legal, mental health, housing, food and others. We maintain an up-to-date registry of referrals for specialty healthcare needs, dental, eye care, shelters and more, and we work to support clients in obtaining the care they need.
Counseling
Individuals are provided with a safe space to share their experiences, and a counselor can help them to create a plan to address issues in their lives. Many clients are referred to SLHP's Mental Health Workshops for further support.
Counselors also help clients navigate the healthcare and community service system to secure specialized services like addiction treatment or support groups.
