Position Summary: Responsibilities include conducting needs assessments, making appropriate referrals to social service agencies, and completing online application for programs. Case managers will document appropriately in the medical records. Case managers will also be able to assess for domestic violence and mental health needs.

Our mission is to serve and advocate for the medically underserved residents in Centre County.
Description
We are a free primary healthcare facility for qualified uninsured residents of Centre County. We offer primary medical and dental care, medication assistance and case management to our clients at no cost. Our services are provided primarily by volunteers,both clinical (physicians and nurses) and non-clinical (receptionists, medical records, eligibility).
