So many of our best memories involve spending the holidays with our loved ones. It might be watching fireworks with young children on the Fourth of July or sharing a Christmas morning in church with a grandparent. You have the power to bring a small bit of this holiday spirit to hospice patients in the Greater Jacksonville Area. Heartland Hospice will be creating special celebrations to occur at local nursing homes and assisted living facilities in which our patients reside. Join us in creating joyful memories for those facing end of life challenges.

Description

Heartland Hospice is Northeast Florida's alternative to meet the growing need for end-of-life care . Our program takes the comprehensive approach to providing quality care for those seeking hospice services including expert medical personnel, spiritual care and bereavement services.

Our volunteers are the cornerstone of our care. Their loving and generous contribution of time and compassion is what makes the difference in our patients' lives.
