Chronic Disease Self-Management Program (CDSMP), known in Utah as Living Well with Chronic Conditions (LWCC), is an evidence-based program developed by Stanford University’s Patient Education Research Center. It is a 6-week workshop that takes place once a week for 2 hours, in groups of 10-15 individuals. It is facilitated by two trained leaders.
This workshop helps participants develop the skills needed to manage chronic conditions, and helps increase life satisfaction for those living with chronic conditions . The subjects covered include: behavior change, strategies to enable participants to manage their medications, ways to increase physical activity levels, effective communication skills, nutrition, decision making, and techniques to deal with problems such as frustration, fatigue, pain, and isolation.
After attending a 4-day training led by Master Trainers, Peer Leaders teach the 6-week LWCC class to community members with chronic conditions (and/or their caretakers) using a detailed scripted manual.

Description
The Davis County Health Department's Senior Services Programs provide a variety of services to the growing population of senior citizens throughout the county. From the senior activity centers, to case management of health-related needs, to providing meals to eligible seniors, the staff of the Davis County Health Department's Senior Services Programs diligently works to maintain and improve the quality of life for its clients.
